To obtain a replacement Member Badge, fill out this form and submit it online.  The replacement fee for a new Member Badge is $26.  When you submit a badge replacement request, the lost badge is deactivated immediately to prevent unauthorized entry into Club property.

This form must be submitted electronically, but before doing so, be sure to verify that all of your Member information in our member database is up to date. Your replacement badge will be mailed to the address we have on file at www.PSCmembers.com.  Verify your information by logging in using your Name, Member No. & birthday.
