To obtain a replacement Member Badge, fill out this form, print it, and mail it with a check for the appropriate amount to:

The replacement fee for a new Member Badge is $25.

You must also submit this form electronically, but before doing so, be sure to verify that all of your Member information in our database is up to date. Your replacement badge will be mailed to the address we have on file. To verify, please go to www.PSCmembers.com and log in using your Name, Member No. & birthday.
